Privacy and Confidentiality

THIS NOTICE DESCRIBES HOW YOUR MEDICAL INFORMATION MAY BE USED AND DISCLOSED AND HOW YOU CAN ACCESS THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

 

1. OUR COMMITMENT TO YOUR PRIVACY

Pahl Pharmaceutical Professionals, LLC (“we,” “our,” or “the Clinic”) is committed to protecting the privacy and confidentiality of your health information.

We comply with:

- HIPAA

- 42 C.F.R. Part 2

- Oklahoma state laws

 

2. WHAT INFORMATION WE COLLECT

We maintain records including:

- Medical and psychiatric history                     - Lab results

- Substance use disorder treatment              - Counseling services

- Medication (including buprenorphine)     - Billing and insurance records

- Telehealth communications                            - Clinical trial participation

 

3. HOW WE USE AND DISCLOSE INFORMATION

Treatment:

We share information with providers, pharmacies, labs, and care teams as permitted by law.

Payment: We share information with insurers, Medicaid, and billing services.

Operations: We use information for quality improvement, compliance, and training.

Clinical Research: Participation is voluntary and requires separate consent.

Legal Requirements: We may disclose information when required by law.

 

4. SPECIAL PROTECTIONS (42 CFR PART 2)

Your SUD records are protected. We will not disclose without:

- Written consent

- Legal authorization

- Permitted exceptions

 

This includes your diagnosis, medications, and participation.

 

5. YOUR RIGHTS

You may:

- Access records

    -You have a right to access:

• Your medical records

• Medication lists (e.g., buprenorphine)

• Lab results (UDS)

• Visit summaries

• Diagnoses and treatment plans

• Your Psychotherapy Notes (to include but are not limited to the following) are specifically excluded under HIPAA and remain excluded:

  • Personal notes kept separately by a therapist

  • Process notes (impressions, raw thoughts, speculation)

- Request amendments

- Restrict disclosures

- Request confidential communication

- Revoke authorization

- File complaints

 

6. TELEHEALTH

We use secure HIPAA compliant systems. 

 

7. BUSINESS ASSOCIATES

We work with vendors who must comply with privacy laws.

 

8. OUR RESPONSIBILITIES

We must:

- Protect your data

- Notify you of breaches

- Follow this notice

 

9. CHANGES

We may update this notice at any time.
